The following is a list of the various forms for use with the Domestic Violence Justice Program

To file a request for a temporary restraining order for civil harassment:

Completely fill out the Request for a Temporary Restraining Order for Civil Harassment form. Also fill out the DV600 form. Bring or send these forms to the Round Valley Indian Tribes Court for filing. The Court Clerk will forward them to the Tribal Court Judge for review. The Tribal Court Judge determines if there is sufficient information to order a Temporary Restraining Order for Civil Harassment.

If Judge has determined there is sufficient evidence for a Temporary Restraining Order for Civil Harassment, the Court Clerk will then issue a Notice of Hearing and Temporary Restraining Order for Civil Harassment. This Order will contain a court date for both parties to be present.


To answer a temporary restraining order (either for civil harassment or domestic violence):

Fill out the appropriate Answer to a Temporary Restraining Order and serve it on the other party.

File a copy with the Court Clerk. Provide Proof of Service with the Answer when filing.

To file a request for a temporary restraining order for domestic violence

Completely fill out the Request for a Temporary Restraining Order for Domestic Violence (DV-109).

Also fill out the Description of Abuse form (DV-101). Also complete a DV-600 form. File with the Court Clerk.

The Tribal Court Judge will review the request and if there is sufficient evidence, the Tribal Court Judge will order a Temporary Restraining Order for Domestic Violence (DV-110 (TC)).

The Tribal Court Clerk will prepare a Notice of Hearing (DV-109) and Temporary Restraining Order for Domestic Violence (DV-110 (TC)).

This Notice of Hearing will contain a Court Date. Both parties need to be present at this hearing. The Court Clerk will forward copies to Round Valley Indian Tribe’s Public Safety for service.

Also the Court Clerk will forward a copy to the Mendocino County Superior Court for filing in that court under the Family Law Division. This will enter the Temporary Restraining Order into the CLETS system for access through all law enforcement agencies.
